2011 CSA MEMBERSHIP APPLICATION/AGREEMENT

  • Total cost of each 12 week Basic Produce CSA membership is $240;
  • A $50 deposit is required to reserve membership for the second 12 weeks; final $190 balance will be due by July 30, 2011; all shares must be paid in full before pick up begins;
  • A signed copy of this application, along with deposit, is required to reserve share;

    • Unless other payment arrangements are made:
  • Checks for Davidson and Lincolnton locations should be made payable to Suzanne Ballard and mailed, along with a copy of this signed application, to 1907 N. Aspen Street, Lincolnton, NC 28092;
  • Checks for Charlotte location should be made payable to Alison Holmes and mailed, along with a copy of this signed application to 6028 Freedom Drive, Charlotte, NC 28214
  • Checks made out to any other payee will be returned to sender and will not guarantee reservation. Any application received without deposit or payment will not guarantee reservation.
      • Pickup information:
      Share pick up: Davidson: Farmer's Market Saturdays 8 a.m. until Noon
      Lincolnton: Downtown Farmer's Market Tuesdays or Thursdays 8am until noon;
      Charlotte location: 6028 Freedom Drive 5-8pm Fridays or 8-10am Saturdays .

      New Moon Farm CSA Contract/Terms of Agreement (Policies and Conditions)

      I understand that the operators of New Moon Rising CSA, pledge to do their best to provide members with the fruit, vegetables, flowers and herbs proposed at their estimated seasons and of the highest quality possible. However, as a shareholder in both the bounty and the risk inherent in any farming venture, I understand that nature ultimately decides what I will receive and when I will receive it. In the unlikely event of dissolution of the CSA by the operator, refunds would be issued on a pro rata basis within 60 days of date of dissolution. Furthermore, I understand that:

      • Second 12 weeks will tentatively run from August 6 thru October 22, with make ups made on or after October 29th.
      • Value added options are fully payable at the time specified by operator.
      • Anyone splitting a share must do so on their own. CSA operator will not be responsible for payment or distribution when a share is split between any parties. Likewise, operator will not be involved in disputes between parties splitting shares.
      • Member shares will be picked up at specific location for the life of the share.
      • Member shares that are not picked up will not be refunded or made up, unless it is the fault of the operator or if notification is made to operators 24 hours prior to pick up time. More than two missed pick ups, even with notification, will result in revocation of the make up priviledge.

      Refund Policy

      • After July 15th, 2011, member becomes responsible for full payment of their share. Refunds will be made only if written notification (or via email) is received by midnight on July 15th, 2011. Refunds will be made within 30 days of this date only;
      • If any member is unable to complete their membership obligation at any time after this date, member has the option of transfer of ownership of their share;
      • Transfer ownership of any CSA share can be made only with prior notification to the CSA operators. New owner must submit a signed a copy of this application BEFORE transfer of share is finalized;
      • Any unpaid balances due from original member must be made to operator before any new member will be accepted;
      • Financial arrangements for sale of any fully paid share is the solely the holder's responsibility.

      I have read and agree to the terms and conditions of the New Moon Rising 2011 Farm CSA. By signing this agreement, I am entering into a partnership with the CSA operator and pledge my support while a member. Furthermore, I understand that while I will participate in the bounty of the participating CSA farms, I also agree to accept the risks inherent to any farming venture such as crop failure, extreme weather conditions, etc. Signed_________________________________________Date_________________________
