Total cost of 12 week CSA membership is $240
A 50% deposit ($120) is required to reserve membership, balance due by Mar. 15, 2010
A signed copy of this application, along with the deposit, is required to
reserve any share
Checks should be made payable to Suzanne Ballard. Mail, along with signed application,
to 1420 Belk Rd., Mt. Ulla, NC 28125
Checks made out to New Moon Farm will be returned
to sender and will not guarantee reservation. Any application received without deposit or payment
will not guarantee reservation
Pickup information:
Pick up will be at the Davidson Farmer's Market Saturdays 8 a.m. until Noon
First pick up is Saturday, May 8, 2010.
New Moon Farm CSA Contract/Terms of Agreement (Policies and Conditions)
I understand that the farmers, David and Suzanne Ballard,
pledge to do their best to provide the fruit, vegetables,
flowers and herbs they propose at the times they estimate and of the highest
quality possible. However, as a shareholder in both the bounty and the risk,
I understand that nature ultimately decides what I will
receive and when I will receive it. In the unlikely event of dissolution of the CSA by the operators, refunds would be issued on a pro rata basis
within 60 days of date of dissolution. Furthermore, I understand that:
Share will run for 12 weeks from May 8th thru July 31 (No pick up July 3rd) .
Because payment of deposits allows the Farm to prepare for the upcoming season,
refunds of any deposit or advance payment will only be made if notification is made by March 1, 2010 (see refund policy below).
Anyone splitting a share must do so on their own. New Moon will not be responsible for payment or distribution
when a share is split between any parties. Likewise, New Moon Farm will not be involved in disputes between parties splitting shares.
Member shares will be picked up at specific location for the life of the share.
Member shares that are not picked up will not be refunded or made up, unless it is the fault of the operator or if notification is made to operators
24 hours prior to pick up time. More than two missed pick ups, even with notification, will result in revocation of the make up priviledge.
Refund Policy
After March 1, 2010, member becomes responsible for full payment of their share. Refunds will be made only if written
notification (or via email) is received by March 1, 2010. Refunds will be made within 30 days of this date. If any member is unable to complete their membership obligation
at any time, after this date, member has the option of transfer
of ownership of their share. Transfer ownership of any CSA share can be made with prior notification to the CSA operators. New owner must submit a signed a copy of this application
BEFORE transfer of share is finalized. Unpaid balances must be made to operators before any new member is accepted.
Financial arrangements for sale of any fully paid share is the solely the holder's responsibility.
I have read and agree to the terms and conditions of the New Moon Farm Organic CSA. By signing this agreement, I am entering into a partnership with the CSA farm and pledge my support to the farmers while a member. Furthermore, I understand that while I will participate in the bounty of the Farm, I also agree to accept the risks inherent to any farming venture such as crop failure, extreme weather conditions, etc.
