• Month-to-Month share is not available as 100% organic or in May or June, no exceptions.
• Payment is due on the 15th of the month prior to each month of participation. For example, if month of participation is July, payment in full must be received by the 15th of June. No late payments will be accepted.
• Month-to-Month share is available for July through October. There is no minimum number of months for participation during that time.
• A copy of this application, along with a signed copy of CSA Agreement, are required before commencing any share pick up.
• Checks should be made payable to Suzanne and Dave Ballard and mailed to 1420 Belk Rd., Mt. Ulla, NC 28125.
  Any checks made out to New Moon Farm will be returned.

Share Pickup:
I will pick up at the Davidson Farmer's Market Saturdays 8 a.m. until Noon.
First Saturday pick up May 2, 2009
I will pick up at the Huntersvile Farmer's Market Tuesdays 8 a.m. until Noon.
First Tuesday pick up May 5, 2009
I will pick up at the Farm Wednesdays 9 a.m. until 5pm.
First Wednesday pick up May 6th, 2009
